Professional Dancer Free Clinic

MDMF offers free consultations for dancers at the The Cowles Center for Dance & the Performing Arts. Dance medicine physical therapists are available in the MDMF Free Clinic most weeks during the year to screen dancers for orthopedic injuries.

  • Appointment times are typically 30 min in duration. If you need more time please feel free to email megin@mndancemed.org to request additional time. Your request will be forwarded to the physical therapists currently staffing the clinic.

Consent form & Privacy notice

Prior to being evaluated or treated at The Cowles Center for Dance & the Performing Arts, you must have a signed consent form and privacy notice acknowledgement form on file. Please note that if you are a minor, a parent or guardian must sign and date these forms prior to evaluation and treatment. A copy of our current privacy notice can be found here. You can print off the Consent Form and Privacy Notice Acknowledgement Form to complete prior to your appointment.
